Updated Application for Initial SAIL Oxygen Funding Forms
 

The initial and palliative home oxygen applications have been revised with two changes to the header:

SAIL’s fax number has been removed – please ensure that requests for initial home oxygen are submitted to the home oxygen company chosen by your patient/client.

The phone numbers have been corrected – there was an error – the number ending in 7121 should be (306) 787-7121. 

Please replace all of your digital and printed copies with this new form.

Testers - What to do with your oxygen testing results

For timely treatment, results that meet criteria should be faxed DIRECTLY to the client's choice of OXYGEN COMPANY. 

The home oxygen company will ensure the SAIL Home Oxygen applications are signed by the prescribers (MD or NP) and forwarded to SAIL. If the tester has easy access to the prescriber for a signature, testers need to ensure both the results and signed SAIL application are faxed to the home oxygen company.

DO NOT SEND RESULTS DIRECTLY TO SAIL.

If more urgent service is required (ie. from hospital or emergency room discharge), call the home oxygen supplier directly to arrange prompt delivery of oxygen.

For results that do not meet criteria anymore, these should also be forwarded to the home oxygen company AND the physician for discontinuation.

If you receive an error code 907, it likely means the internal battery is dead. Reach out to your local biomed contact to have this replaced.
